We are based in Dandenong and provide supports across Melbourne and parts of Victoria. Our focus is on delivering reliable, respectful and person-centred services that help participants feel supported in their everyday lives, at home and in the community.
Care Hub 24/7 was created to provide support that is done properly. We believe support should be participant-led, not provider-driven. Every person has their own goals, routines, preferences and vision for their life, and our role is to walk alongside them while helping them lead their own journey.
Our current services include community supports, personal care, daily living assistance and accommodation supports. Whether we are supporting someone to access the community, maintain their independence at home, build daily living skills or receive support within an accommodation setting, our approach remains the same: provide care that is consistent, practical and built around the person.
We are committed to upholding choice, independence and dignity in every area of support. We believe participants should be heard, respected and actively involved in decisions about their care and daily life.
Good service comes from strong communication, reliable staffing, clear oversight and genuine connection. Our goal is to support participants with confidence and consistency while building trust with families, support coordinators and the wider care network.
